If you have already been approved to vote by mail in 2025 but need your ballot mailed out to a different address than the one on your application, you will need to complete the Request to Change Ballot Delivery Address form.
This form can be completed and returned online, simply follow the instructions on the form. You can also download and print the form, if you prefer.

This form only changes the address your ballot is being mailed to. If you have moved, or are permanently living at a new address, you will also need to update your Voter Registration.
Ballots are not forwarded by the USPS.
